If you require any vaccinations relating to foreign travel you will need to complete a form for each traveller, including children, which can be downloaded below or collected from reception. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.
Travel Risk Assessment Form
There is further information about countries and vaccinations required on the links below
It is important to contact the surgery as early as possible - at least 6 weeks before you travel - as an appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Further information can also be found online : www.fitfortravel.nhs.uk
If you have left it too last minute to receive vaccines from ourselves you can contact Charles Michies Pharmacy which is located at 391 Union Street, Aberdeen on 01224 585312.
Travelling in Europe
The European Health Insurance Card (EHIC) can be used to cover any necessary medical treatment due to either an accident or illness within the European Economic Area (EEA). The EHIC entitles the holder to state-provided medical treatment within the country they are visiting and the service provided will be the same as received by a person covered by the country’s ‘insured’ medical scheme.
The EHIC is the replacement for the E111. As of 1 January 2006, E111’s are not valid.
